Position Summary
Pharmacist: The Antimicrobial Stewardship Pharmacist is an integral member of the NYGH Antimicrobial Stewardship Program and a key player in establishing initiatives that optimize antimicrobial use with a focus on patient safety and quality improvement. The Pharmacist will have both a direct patient care role and an institutional monitoring and evaluation role to ensure antimicrobials are used appropriately and effectively according to hospital guidelines and best practices identified in the infectious diseases literature.
On a practical level, you will
  • Develop, implement, and expand, on current institutional antimicrobial stewardship initiatives (e.g. Prospective audit and feedback, restricted antimicrobial use, intravenous to oral step-down, clinical guidelines and pathways).
  • Clinical resource for infectious diseases medication management (e.g. beta-lactam allergy assessment, complex medication counselling)
  • Collaborate with key departments (e.g., Pharmacy, Decision Support, Clinical Informatics, and Infection Control) in collecting and monitoring antimicrobial utilization metrics, leveraging information systems in the development of clinical decision-support tools, and monitoring antibiotic resistance trends to guide empiric therapy.
  • Plan and prepare program updates for the Antimicrobial Stewardship Committee and other key stakeholders or committees.
  • Participate in the review of antimicrobial formulary management
  • Participate in the preparation of reports intended to document the impact of the program on quality, safety, and cost of antimicrobial utilization
  • Assist in developing, organizing, managing, and querying a newly developed NYGH ASP metric database
  • Provide education, information, didactic and experiential training about antimicrobial stewardship to students, physicians, pharmacists, community partners and other key members of the health care team.
  • Participate in research to advance the field of antimicrobial stewardship
Qualifications
QUALIFICATIONS
  • Previous training or experience in antimicrobial stewardship program or infectious diseases (e.g MAD-ID, SIDP certification)
  • Advanced Pharmacy Residency Program (Year 2) in Infectious Diseases is an asset
  • Board Certification in Infectious Diseases Pharmacist is an asset
  • Pharmacist Injection training
  • Advanced computer skills including database management and software programming.
  • Experience in epidemiological research and/or statistics. MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
  • Graduate from an accredited university Pharmacy Program (Bachelor Level or Doctor of Pharmacy).
  • Licensed under Part A of the Ontario College of Pharmacists.
  • Previous clinical hospital experience or hospital residency is required. SKIILS REQUIRED:
  • Knowledge of pharmaceutical care process, disease management and clinical pharmacology.
  • Demonstrated ability to work within a team structure with strong collaborative skills.
  • Demonstrated critical thinking and deductive reasoning skills using evidence-informed practice.
  • Demonstrated experience in establishing relationships with key stakeholders and opinion leaders pivotal to advancing patient care and enhancing quality performance.
  • Experience with data analysis and effective monitoring and evaluation of implemented programs.
  • Adaptability and flexibility in a dynamic environment.
  • Demonstrated ability to exercise independent judgment and to assess situations and problems efficiently and effectively.
  • Effective interpersonal, organizational, and verbal and written communication skills.

Working at NYGH means working with a dynamic team of fellow healthcare providers, staff, and volunteers in one of Canada’s leading hospitals. This is a full time permanent position with 8 hour shifts Day shifts, Evening shifts, including weekends in Pharmacy Services. We offer a highly competitive total compensation package that includes pension and vacation.
