Key Duties

• Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
• Uphold a prescribing qualification, or training to become a prescriber, and will work with and alongside the multi-disciplinary team across the Primary Care Network.
• Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex poly-pharmacy, especially the elderly, people in care homes, those with multiple long term conditions (in particular COPD and asthma) and people with learning disabilities or autism (through STOMP - Stop Over Medication Programme).
• Provide specialist expertise in the use o medicines while helping to address both the public health and social care needs of patients in the network.

• £50,000-£55,000 dependent on experience and skill set (negotiable)

Key Skills, Qualifications And Client Requirements

• Essential Vocational Masters Degree in Pharmacy (4 years or equivalent) \+ pre-registration training \+ post registration training relevant to general practice Member of the General Pharmaceutical Council and/or Royal Pharmaceutical Society Desirable PgCert
• Pharmacist Independent Prescribing Qualification or equivalent
• Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. UK Registration
